You have to create a survey AFTER you fill out a survey. They don't give you the diffuser after just filling out the survey.
I dont see where it says that. I think after you fill out the survey it will ask you if you want to create a survey because it is promoting the company survey monkey. I filled out my address and should be expecting one soon. :) Hopefully. :)
